CureSearch National Childhood Cancer Foundation and Children's Oncology Group are partners in the search to cure childhood cancer, each with vital responsibilities concerning research, care, public awareness and fundraising.
About CureSearch

Under normal conditions patient registration and study enrollment occurs via the secure COG member's website. In the event the website is unavailable (unscheduled outage) for an extended period of time (generally more than 4 business hours) and there is an urgent need to enroll or randomize a patient, the following procedures should be followed.

If the patient does not have a COG ID number:

  • Complete the following form and fax it to (626) 445-4334
  • Staff at the Statistics & Data Center will perform the transaction and will send you confirmation and the COG ID number assigned to the patient.

If the patient already has a COG ID number:

  • Contact the COG helpdesk (626) 447-1674 and request a copy of the Eligibility CRF for the study. It will be sent to you via fax or email.
  • Complete the form and return it
  • Staff at the Statistics & Data Center will perform the transaction and will send you confirmation and the COG ID number assigned to the patient.

This information will be posted to the COG public website, the CureSearch website and the COG member's website.
